This is recent behavior. About a month or so ago, I realized that mp3tag wasn't downloading folder.jpg and setting it in the Cover Art area. It doesn't matter if I'm trying to get art from Amazon or MusicBrainz.
I do see it in the preview area, but when I click Ok, there is no artwork.

In the web source dialogue that shows the tracks, check the utils button at the bottom left and see the options there.
And: what happened a month ago? Did you migrate to another OS? Then check the access rights and ownership of the folders with the music
It took me a few minutes to find the dialogue box, but that was the problem! I have no idea how those got unchecked...
