I'm getting an issue when adding covers where the cover appears as a small thumbnail in the upper left and the rest of the square is black.
I've tried with multiple items with the same result.
I've tried dragging/dropping, importing and right-clicking and adding the file all with the same result.
I don't remember what version I had previously, but I just updated to v2.72 when this issue manifested.

It is just a shoot into the dark:
look into the right click properties of the shown cover image ...
and try out the option "Correct aspect ratio".

What kind of file?
Portable installation or local one?
What kind of device? Mobile phone/mobile network or landline?
Does the preview for such files work for other programs?
