Hey, just so you know, you can export the cover of a specific album by right-clicking on it.
At the moment, FOLDER is always suggested as the file name, which is great! I'd really love to be able to use 'COVER' as the default name, if that's ok? Do you know where I can set this?

You mean the context menu entry "Extract cover"?
If you want the fixed output suggestion COVER as file name for every new extracted cover, you can set it like that:

This does not only apply to the first cover (as your thread title slightly misleadingly suggests).
You can Extract every cover that you see in your album.
Just use the navigation arrows under the coverart picture:
(only visible if more than 1 image is embedded)
The right click context menu works for every selected and visible picture.
