Small CD-Back?

Hi folks,
I'm looking for a small CD-back-plugin...something like HTML_Vitamine_CD, but with 'Tracknumber-Track-ARTIST-length' included. In HTML_Vitamine_CD there is no artist displayed...and all the other plugins I've found are too big/too much info...
Its only needed for my CD-flipcase (no hardbox).

It Should have a simple look like this:

- ALBUM-NAME - (maybe the year here) 1. - Track XY - Artist XY - (min:sec) 2. - Track XY - Artist XY - (min:sec) 3. - Track XY - Artist XY - (min:sec) 4. - Track XY - Artist XY - (min:sec)

or this:

  • ALBUM-NAME -
    (maybe the year here)
    • Track XY - Artist XY - (min:sec)
    • Track XY - Artist XY - (min:sec)
    • Track XY - Artist XY - (min:sec)
    • Track XY - Artist XY - (min:sec)

Is there somthing like this to get here?
