As I just had the opposite experience, the function cannot be all wrong.
So it would help if you could tell us what kind of file it is, whether the file has been tested for integrity (which does not mean "it plays"), see e.g. here:
And is this a new file that you just got or an old one that has worked before but now you cannot add pictures?
