Why %_path% not work in this tool?
Maybe (untested!) you need to write
MTTOOLSPARAM=/o /p=r /r="%%_path%%"
or
MTTOOLSPARAM=/o /p=r /r=%_path%
also this suggest not work.
Please show us a screenshot from your above settings window.

The problem occurs if you manipulate the tools.ini file directly.
(You should not change it directly!)
If you enter the parameters in Mp3tag for Tools like this:
/o /p=r /r="%_path%"

Mp3tag translate it automatically and reusable to
MTTOOLSPARAM=/o /p\u003dr /r\u003d"%_path%"
Just for completeness:
The used command line parameters for Total Commander are
/O If Total Commander is already running, activate it and pass the path(s) in the command line to that instance (overrides the settings in the configuration dialog to have multiple windows)
/P= Sets the active panel at program start: /P=L left, /P=R right. Unfortunately this does not work (v6.53) with the Parameter /O currently.
